If you are interested in receiving 1.5 continuing education credits, please:

1. Preregister on the Minority Health Project web site; you will be asked to create a five-character participant code consisting of any combination of numbers or letters.
2. Write your participant code and the CE code that will be announced immediately prior to each presentation on the evaluation form distributed by the facilitator at your site.
3. Complete and return the evaluation form at the end of each day's session. Your evaluation form must contain your participant code and the CE codes for the two presentations. (Note: since forms were printed before this procedure was designed, the forms do not refer to these codes.)
4. You may pay the $4.00 fee by telephoning the UNC SPH Office of Continuing Education at 919-966-4032 or 919-966-1104 and providing a credit card.
5. Upon receipt of the fee, the Office of Continuing Education will send your continuing education certificate to you.
