Instructions:
A. The answers you submit with this form will go into a database and will be sent by email to Vic, to your TA (for case studies), and to you. You do not need to submit all your answers at the same time.
B. Please review the email you receive. You may change an answer by resubmitting it.
The new email will show only the resubmitted answers but the previous submissions will remain in the database.
C. Please compose, spell check, and review your answers
in your word processing program. Then copy and paste each answer into the appropriate location in the submission form.
D. Unfortunately we can only accommodate plain text (i.e., no bolding, underscores, italics, and color). If you want to include a table it must be laid out in regular text, e.g.
